GUIDELINES FOR FILLING IN THIS FORM (NUMBERS REFERRING TO THE VARIOUS BLOCKS):

The applicant will choose the details concerning his study in China according to the clues given in this form. However Confucius Institute Headquarters is entitled to make any necessary adjustments according to the applicant’s preferences.

第2篇: 獎學金申請表個人總結
單項獎學金申請表（附件1）
北京交通大學雙培計劃學生獎勵辦法（附件3）針對我校雙培計劃學生，學校擬制定如下獎勵辦法：
一、基本條件
雙培計劃學生申請個人獎勵需滿足以下基本條件：
1.熱愛祖國，擁護中國共產黨，愿意為社會主義現代化建設服務；
2.遵守國家法律法規和校規校紀，品行端正，無違法違紀行為；
3.學習勤奮、刻苦，學習成績優良，即上一學年學習加權平均成績在60分以上；
4.學年思想行為測評成績居雙培計劃學生內部專業排名前70%；
5.上一學年宿舍個人得分平均分達到60分以上（含60分）。
二、單項獎學金評定
1.學習優秀獎學金
雙培計劃學生可與我校正常學籍學生共同參評學校設立的學習優秀獎學金，一、二、三等學習優秀獎學金評定及獎勵標準保持不變。如雙培計劃學生獲評，則為其所在學院追加相應等級的學習優秀獎學金名額。
同時，設立雙培計劃單項學習優秀獎學金，對未能獲得學校一、二、三等學習優秀獎學金，但學習加權平均成績在雙培計劃學生內部專業排名前25%的，可獲得雙培計劃單項學習獎學金。學習成績以教務部門提供的數據為準，要求無不及格現象。我校學習優秀獎學金與雙培計劃單項獎學金不可兼得。雙培計劃單項學習獎學金共分兩檔：
（1）雙培一等學習優秀獎學金：學習加權平均成績在專業排名前12.5%，獎勵金額1000元/人?學年；
（2）雙培二等學習優秀獎學金：學習加權平均成績在專業排名前25%，獎勵金額800元/人?學年。
2.其它類單項獎學金類
雙培計劃學生可與我校正常學籍學生共同參評學校設立的其它類單項獎學金，包括社會工作優秀、奮進、學習進步、體育活動優秀、文藝活動優秀、社會實踐優秀等類別獎學金，評定標準保持不變。如有雙培計劃學生獲評某類別單項獎學金，則為其所在學院追加相應類別的獎學金名額。
三、榮譽稱號評定
1.雙培計劃學生獲評“三好學生”榮譽稱號需滿足以下條件：
（1）學年思想行為測評成績居雙培計劃學生內部專業排名前30%；
（2）獲得學校一、二、三等學習優秀獎學金或獲得“雙培”一等學習優秀獎學金；
（3）上一學年體質測試成績達到60分以上（含60分）。
2.雙培計劃學生獲評“優秀學生干部”榮譽稱號需滿足以下條件：
（1）學年思想行為測評成績居雙培計劃學生內部專業排名前30%；
（2）獲得二等及以上社會工作優秀獎學金；
（3）上一學年體質測試成績達到60分以上（含60分）。
四、專項獎學金評定
1.雙培計劃學生可與我校正常學籍學生共同參評學校現有的專項獎學金，評定標準保持不變，即：
（1）榮獲校級及以上三好學生或優秀學生干部稱號者；
（2）符合專項獎學金提供者要求的專項條件。
2.為雙培計劃學生單獨設立“知行”雙培專項獎學金，評選條件為榮獲校級及以上三好學生或優秀學生干部稱號，獎勵金額5000元/人?學年。
五、國家獎學金評定
雙培計劃學生在原學籍所在學校評選國家獎學金，我校對雙培計劃學生的表現進行評定，由其學籍所在學校依據相關辦法評選國家獎學金。
